What We Treat

It's Not "Just PMS," and You're Not Overreacting

For one to two weeks before your period, you feel like a different person: hopeless, furious, anxious, or completely overwhelmed. Then your period starts, and within a few days you feel like yourself again. If that cycle repeats month after month, you may be dealing with premenstrual dysphoric disorder (PMDD), a recognized mood disorder that is highly treatable.

PMDD symptoms typically appear in the luteal phase (after ovulation) and can include:

  • Severe irritability, anger, or conflict with people you love
  • Depressed mood, hopelessness, or crying spells
  • Intense anxiety or feeling on edge
  • Mood swings or sudden sensitivity to rejection
  • Fatigue, trouble concentrating, and changes in sleep or appetite
  • Physical symptoms like bloating, breast tenderness, or headaches

What Is the Best Medication for PMDD?

The best PMDD medication depends on your symptom pattern, health history, and preferences. That’s why we start with a careful evaluation, often including a few cycles of symptom tracking, to confirm PMDD and distinguish it from depression or anxiety that simply gets worse before your period (called premenstrual exacerbation, which is treated differently).

SSRIs are the most established first-line treatment for PMDD and often work faster for PMDD than they do for depression. Some patients take them daily. Others take them only during the luteal phase of their cycle. Your provider will help you decide which approach fits your life, then track how you feel across cycles and adjust as needed.

If SSRIs aren’t the right fit, other options may be considered, and we’ll coordinate with your OB/GYN if hormonal treatment becomes part of your plan.

Frequently Asked Questions

How is PMDD different from PMS?

PMS is common and usually mild. PMDD causes severe emotional symptoms that disrupt work, relationships, or daily functioning, and it's classified as a depressive disorder.

Can I take medication only part of the month?

For many patients, yes. Luteal-phase dosing is an established option your provider can discuss with you.
