Your First Step Toward Personalized Care
New Patient Information
Starting psychiatric care can feel like a big step. At Synchronous Mental Health, a psychiatric nurse practitioner meets with you for a thorough first visit, listens to your history and goals, and builds a plan with you. This page explains who we see, what your first appointment involves, and how to get ready.
We are a psychiatric practice based in Portland, Oregon. We provide psychiatric evaluation and medication management for adults, and we offer telehealth visits to patients in Oregon, Washington, Florida, and California.
- Your first visit is a comprehensive psychiatric assessment, held by secure video.
- You will see the same provider for your follow-up visits.
- If medication is right for you, we build your plan with the fewest medications necessary.

What Happens at Your First Appointment
Your first appointment is a psychiatric assessment. Your provider will ask about what brings you in, how long it has been going on, and how it affects your daily life. They will also ask about your medical history, any medications or supplements you take, past treatment, sleep, substance use, and your family history. You are welcome to ask questions at any point.
By the end of the visit, you and your provider will talk about what may be going on and what your options are. Depending on your situation, that may include medication, therapy referrals, lifestyle changes, or additional testing. You decide what feels right, and your plan can change as you go.

How to Prepare
A little preparation helps you get more from your visit:
-
List your medications. Include doses, supplements, and anything you take over the counter.
-
Write down your symptoms. Note when they started, what makes them better or worse, and how they affect work, sleep, and relationships.
-
Gather past records if you have them. Prior diagnoses, medication histories, or lab results are helpful.
-
Think about your goals. What would you most like to be different?
-
Check your insurance details. Have your card handy, and see our insurance and pricing page.
Telehealth Visits
Telehealth lets you meet with your provider from home or another private space. You will need a device with a camera, a reliable internet connection, and a quiet place to talk. Because of state licensing rules, you need to be physically located in Oregon, Washington, Florida, or California at the time of each appointment.
If you live in Portland, ask our office about in-person options.
Ongoing Care
After your first visit, you will have regular follow-up appointments with the same provider. These visits are a chance to check how you are feeling, review how any medication is working, and adjust your plan. Many people find that having one provider who knows their history makes care feel more personal and more effective.

Insurance
We accept most major insurance plans, and our team can help you understand your coverage before you begin. See our insurance and pricing page for details.
If You Are in Crisis
Our practice is not an emergency service. If you are in crisis or thinking about harming yourself, call or text 988, or call 911. You can find more options on our crisis and emergency resources page.
What to Expect From Your Provider
Our psychiatric nurse practitioners listen first. They want to understand not only your symptoms but also your life: your work, your relationships, your sleep, and what you have already tried. You can expect clear explanations, time for questions, and respect for your preferences. If something is not working, tell your provider. Treatment is a partnership, and your plan can change as you learn what helps.
